Most children seen for ear pinning surgery are between seven and twelve years old. By age seven the ear has reached close to its adult size, so the result is stable, but the cartilage is still soft enough to reshape with low force. The operation in children is usually performed under general anaesthesia so the child is fully asleep. The procedure itself takes one to two hours for both ears. A bulky bandage is placed around the head straight after surgery and stays on for about a week. Most children are back at school within a week, and most contact sports and swimming are paused for three to four weeks while the cartilage settles into its new position.

In adults the procedure can usually be performed under local anaesthesia with sedation, which means the patient is relaxed and comfortable but not fully asleep. General anaesthesia is also an option for adults who prefer it or where the cartilage is firmer and a fuller plane of work is needed. The choice is made between the patient, Dr Konrat, and the anaesthetist during the two required consultations.

Adult cartilage is firmer than children's cartilage, which sometimes means slightly different technique choices during the operation. The healing time and the post-procedure routine — bandage for a week, lighter headband for a few more weeks, no contact sport for three to four weeks — are similar to those for children.
