Mastopexy

It removes excess skin, repositions the nipple-areola complex to a higher position, and tightens the surrounding tissue. The aim is to restore a more anatomically-natural breast shape — not to change breast size, although volume can appear different after the lift because of the change in shape.
Breast lift surgery is performed for ptosis — the medical term for sagging — that has developed after pregnancy, breastfeeding, weight loss, or simply through the natural loss of skin elasticity over time. The procedure does not place implants. Grades of Ptosis &
What Each Means
Ptosis is graded by where the nipple sits relative to the inframammary fold (the crease beneath the breast). The grade determines what technique is appropriate and what scar pattern results.
Grade 1 — Mild ptosis
Nipple at the level of the inframammary fold or just below. Often addressed with a smaller technique (periareolar) when skin removal needed is limited.
Grade 2 — Moderate ptosis
Nipple is 1–3 cm below the inframammary fold but still above the lowest contour of the breast. Generally requires a vertical (lollipop) lift pattern.
Grade 3 — Severe ptosis
Nipple is more than 3 cm below the inframammary fold and may sit at the lowest point of the breast. Typically requires a wise-pattern (anchor) lift, which has a longer scar but provides the most reliable lift for severe cases.
Pseudoptosis
Where the breast tissue itself has dropped but the nipple position relative to the fold is normal. Pseudoptosis is sometimes addressed with a different operative plan than true ptosis. Dr Konrat assesses this carefully at consultation because the right technique depends on the actual pattern, not just the visual impression.
Surgical Techniques —
What the Scar Looks Like
Three main lift patterns are used. The choice depends on the grade of ptosis, the amount of skin to be removed, and the breast shape.
Periareolar (donut) lift
A circular incision around the areola only. Suitable for mild ptosis where modest skin removal is enough. Lowest scar burden but provides limited lift.
Vertical (lollipop) lift
A periareolar incision plus a vertical incision running from the areola down to the inframammary fold. Suitable for moderate ptosis. Provides more lift than the periareolar approach with a scar that is well hidden when wearing standard clothing.
Wise-pattern (anchor) lift
A periareolar incision, a vertical incision, plus a horizontal incision along the inframammary fold. The longest scar pattern but provides the most reliable lift for severe ptosis or large volumes of skin to be removed.

Process
GP Referral & First Consultation
Obtain a GP referral. Attend your first consultation at the Bondi Junction practice. Dr Konrat reviews your medical history, examines the breasts, assesses ptosis grade, discusses appropriate techniques, and explains the realistic outcomes and risks.
Second Consultation & Cooling-Off
Attend the second consultation at least a week later. The operative plan is confirmed. The mandatory 7-day cooling-off period follows before the surgery date can be booked. A written quote covering practitioner, anaesthetist, and facility fees is provided.
Pre-Operative Preparation
Stop smoking at least 6 weeks prior. Avoid aspirin and anti-inflammatory medications for 2 weeks. Arrange 2–4 weeks off work, a responsible adult to drive you home and stay the first night, and a supportive surgical bra for recovery.
Surgery Day
Mastopexy is performed under general anaesthetic at an accredited day-hospital facility. The procedure typically takes 2–3 hours. Drains may be placed and removed at the first follow-up. Most patients are discharged the same day.
Recovery & Follow-Up
Rest is needed for the first 1–2 weeks. Follow-up appointments at the Bondi Junction practice monitor healing. Most patients return to desk-based work after 2 weeks and are cleared for exercise from 4–6 weeks. Final scar settling continues over 6–12 months.
What to Expect
After Surgery
Recovery from breast lift surgery is a staged process. Each phase has practical aftercare requirements and activity restrictions that protect the result. Dr Konrat provides written instructions before the procedure date and reviews progress at follow-up appointments at the Bondi Junction practice.
Initial Rest
Rest at home with the chest elevated. A surgical bra is worn day and night. Pain relief is taken as prescribed. Sleeping on the back is required. Drains, if placed, may still be in place during this window.
Early Healing
Most desk-based and work-from-home patients return to work in this window. Drains, if placed, are removed at the first follow-up. Surgical bra is still worn day and night. No lifting above 2 kg, no overhead movements, no exercise.
Continued Settling
Light activity is increased gradually. Walking is encouraged. Surgical bra continues day and night. Most patients can do light physical work but no exercise, no swimming, no lifting children.
Return to Exercise
Most patients are cleared for return to exercise at this point, starting with low-impact activity (walking, light cardio) and building back to normal training over the following weeks. High-impact exercise and chest work is delayed.
Final Settling
Breast shape continues to settle into its final position over 3–6 months. Scar maturation continues over 6–12 months. Final scar appearance is generally reached around the 12-month mark with appropriate scar care.
