A breast lift (mastopexy) is a surgical procedure that repositions the breast tissue and nipple-areola complex to a higher position on the chest wall. During the procedure, excess skin is removed and the remaining breast tissue is reshaped to create a more proportionate breast mound.

Mastopexy is designed to address breast ptosis (sagging) rather than significantly alter breast size. The procedure is planned according to the degree of ptosis, breast shape, skin quality, and the scar pattern that may be required.

Ptosis

Breast ptosis is classified by the position of the nipple relative to the inframammary fold (the crease beneath the breast). This grading system helps determine which surgical approach may be most suitable.

  • Grade I (mild) — the nipple sits at or just below the level of the inframammary fold.
  • Grade II (moderate) — the nipple has descended further below the fold but still faces forward.
  • Grade III (severe) — the nipple sits well below the fold and points downward.

There is also a category known as pseudoptosis, where the nipple remains above the fold but the lower breast tissue has descended below it. Dr Konrat will assess your degree of ptosis and recommend an appropriate technique.

Surgical Techniques

Techniques

Several techniques are used in breast lift surgery, each suited to different degrees of ptosis and breast anatomy:

  • Periareolar (donut) lift — an incision is made around the areola only. This technique is generally suited to mild ptosis and results in a scar around the edge of the areola.
  • Vertical (lollipop) lift — an incision runs around the areola and vertically down to the inframammary fold. This is commonly used for moderate ptosis and allows more tissue repositioning than the periareolar method.
  • Anchor (inverted T) lift — an incision runs around the areola, vertically down to the fold, and along the fold itself. This technique is typically used for more significant ptosis where greater tissue removal and reshaping is needed.

Dr Konrat will recommend a technique based on your anatomy, the degree of ptosis and your goals. The choice of technique also determines the location and extent of scarring.

Aftercare

Scars, Garments &
Drains

Breast lift surgery leaves permanent scars. The exact scar pattern depends on the technique used, but there is always a scar around the areola and often a vertical scar beneath it. Some patients also require a scar along the breast crease.

Drains are commonly placed in each breast during surgery and are usually removed in the first week. A supportive surgical garment is typically worn day and night for 2 weeks, then during the day for a further 3 weeks. Careful scar support and avoiding unnecessary tension on the wounds are important parts of recovery.

Consultation & Referral

A GP referral is required. Two consultations (at least one in person) are needed before surgery. Your medical history, breast anatomy, skin quality, degree of ptosis and nipple position will be assessed. Mandatory 7-day cooling-off period applies.

Pre-Operative Preparation

Dr Konrat will mark the incision lines before the procedure. A supportive surgical bra should be purchased in advance. Plan for a responsible adult to drive you home and stay with you for the first 24 hours.

The Day of Surgery

Performed under general anaesthesia. Takes 2–3 hours depending on the technique used. Drains are commonly placed in each breast and tissue removed during surgery is sent to pathology. Most patients are discharged the same day.

Recovery & Follow-Up

Rest for 1–2 weeks. A supportive surgical bra is worn. Gradual return to activities at 2–3 weeks. Exercise resumes at 6 weeks with Dr Konrat's clearance. Breasts continue to settle over 3–6 months.

What to Expect
After Surgery

Recovery varies between individuals. The following is a general guide. Dr Konrat will provide detailed aftercare instructions specific to your procedure.

Week 1

Rest & Initial Healing

Rest at home. Mild to moderate discomfort, swelling and bruising are expected. Pain medication will be prescribed. A supportive surgical bra is worn.

Week 2–3

Return to Light Duties

Many patients return to light duties or desk-based work. Swelling begins to subside. Avoid lifting anything heavier than a few kilograms.

Week 4–6

Gradual Return to Normal Activities

Gradual return to normal activities. Exercise and strenuous activity can typically resume after six weeks with Dr Konrat's clearance.

Months 3–6

Settling & Scar Maturation

Breasts continue to settle into their new position. Scars will still be visible but will continue to fade over twelve to eighteen months.

Important: Contact the clinic immediately if you experience excessive bleeding, severe pain, or signs of infection. Detailed aftercare instructions will be provided before and after your procedure.
